The calculator is available below in Excel 2007 format or in template format. I strongly recommend using the Template format. Saving it as a template ensures that you will never accidentally save a completed form over the blank spreadsheet. Saving the template in the template folder (on Vista, usually C:Users<your name>AppDataRoamingMicrosoftTemplates) will allow you the choice to create a new document based on the template any time you start a new document.
